Notify of IP address change via email

I have HMAVPN and it sometimes changes the IP address and then I'm no longer able to connect to my home computer because I don't know the new IP. How do I monitor if the PPP HMAVPN adapter changes?

HMAVPN is proxy? Does it provide port forwarding option so you can connect to system behind proxy using some external IP?
This means you actually need this "external IP", not real IP of your home system?
Then I think you should contact developers of this HMAVPN, we don't know what exactly services they provide...
